In order to complete a request for a formula refill, we require the completion of this form. Please note the following:

• If you would like an adjustment to your formula, do not fill out this form. Please have a walk-in with Dr. Lin.
• If you are not sure whether you should get a refill or have your formula updated by having a consultation, please read our FAQs
• We are closed Wednesdays. Requests filled after 3:00pm Tuesday will be fulfilled the following Thursday.
• If you have more than one patient formula that you want refilled, you need to complete this form for EACH patient.
• If it has been a while since you have been seen (6+ months), you may want to consult with the herbalist to ensure your previous formula is correct for the newest manifestation of symptoms. We do consider it poor practice to refill formulas that are no longer appropriate, and therefore we may end up phoning you to update the formula. 
• If the cost of the formula has increased since your last purchase, we will ask for your confirmation before charging your card.
• Pickup orders will be ready within 2 business hours of the form submission. 
• Shipping orders to California will typically take 3-5 days. While shipments within the Tri-state area will take 1-2 days. 
• Shipping and handling costs vary according to distance and weight. Typical refill orders within the surrounding area of NY are $12.50 S&H, while packages to California cost around $17.
